1. Completed application form (non-refundable application fee: $100.00)
2. Psychoeducational Evaluation or Neuropsychological Evaluation (current within one year)*
3. Speech and Occupational Therapy evaluations if applicable for your child*

*Please note: Family Interviews will not be scheduled without current evaluations.
